I wanted to report back what the cause turned out to be since everyone here was so very helpful with attempts to diagnose and troubleshoot.
It turns out that xcache was causing apache do to what it was doing. I disabled all opcode caching and the server was stable for 4 weeks, then enabled APC and it has been stable since.
